Certified Nursing Assistant (CNA)

Position Summary

VenCare Health Solutions is seeking compassionate and reliable Certified Nursing Assistants (CNAs) to provide high-quality in-home support to clients in the comfort of their homes. CNAs assist clients with daily living activities, personal care, and companionship while promoting independence, dignity, and safety.

Key Responsibilities

Personal Care Assistance

• Assist clients with bathing, grooming, dressing, and hygiene

• Help with toileting and incontinence care

• Assist with mobility, transfers, and positioning

• Support clients with ambulation and fall prevention

 

Daily Living Support

• Assist with meal preparation and feeding if needed

• Help with light housekeeping such as laundry, dishes, and tidying living areas

• Provide medication reminders (no administration unless authorized)

• Assist with errands and grocery shopping

​

Companion Care

• Provide companionship and emotional support

• Engage clients in conversation and activities

• Accompany clients to appointments or social outings

 

Monitoring Client Well-being

• Observe and report changes in client condition or behavior

• Maintain accurate care notes and documentation

• Follow individualized care plans and agency guidelines

 

Qualifications

• Current South Carolina CNA Certification

• High school diploma or GED preferred

• CPR / First Aid certification preferred

• Valid driver’s license and reliable transportation

• Ability to pass background check and drug screening

• Previous home care or senior care experience preferred

Registered Nurse (RN) 

Key Responsibilities

Client Assessments and Care Planning

  • Conduct initial and ongoing client assessments

  • Develop and update individualized care plans

  • Evaluate client health status and care needs

  • Communicate with physicians, families, and care teams

​

Clinical Care

  • Monitor and evaluate client health conditions

  • Administer medications as prescribed (when applicable)

  • Perform skilled nursing procedures within scope of practice

  • Provide wound care and dressing changes

  • Monitor vital signs and health indicators

  • Manage chronic conditions and health monitoring

​

Caregiver Supervision

  • Provide clinical supervision of CNAs and PCAs

  • Ensure caregivers follow care plans and safety procedures

  • Conduct caregiver training and competency evaluations

  • Provide guidance and support to caregiving staff

​

Documentation and Compliance

  • Maintain accurate clinical documentation

  • Ensure compliance with South Carolina Department of Health and Environmental Control (DHEC) regulations

  • Maintain HIPAA compliance and patient confidentiality

  • Participate in quality assurance and performance improvement programs

​

Care Coordination

  • Communicate with physicians, therapists, and other healthcare professionals

  • Educate clients and families about care plans and health management

  • Coordinate community resources and services

Qualifications

  • Current South Carolina Registered Nurse (RN) license

  • Graduate of an accredited nursing program

  • Current CPR certification

  • Valid driver’s license and reliable transportation

  • Ability to pass background check and drug screening

  • Home care or community health experience preferred
